Disclosure will take place on a ‘need-to-know’ basis. Only those individuals/organizations who need to know to provide care for you and for the proper administration of Government (whose personnel are covered by strict confidentiality rules) will be given the information.

• The right to request confirmation of the personal data we hold about you and how we are using it.
• The right to request correction of your personal data if it is incorrect, out of date, or incomplete.
• The right to request that we stop any consent-based processing of your personal data after you have withdrawn that consent.

You have the right to access the data we hold about you and to receive a copy. Parents may access their child’s records if this is in the child’s best interests and not contrary to a competent child’s wishes. Formal applications for access must be made in writing to The Practice Manager.
